The inside of the bladder is covered by a thin, mucous GAG layer (glycosaminoglycans). It acts as a barrier that separates the irritating substances in urine from the sensitive cells of the bladder wall. In IC/BPS this barrier can be weakened, so urine irritates the nerves and adds to the pain.

Many of the approaches used in IC/BPS — including D-mannose for infections and barrier-supporting products — revolve around the very idea of protecting this lining. The GAG-layer mechanism is the key to understanding why the bladder “burns” even when no bacteria are present.
